09. Referral Pathways for Mental Health, Addictions & Substance Use Health Urgent Care Centre

Background: Emergency department visits for mental health, addictions, and substance use health are on the rise. Diverting these patients to a center catered to their needs can alleviate challenges to our healthcare system by reducing paramedic offload times and reducing emergency department volumes.


Objective(s): To organize the contributions from our 26 community partners into a systematic referral pathway for patients that visit the Mental Health, Addictions, and Substance Use Health Urgent Care Centre.


Approach: I created an extensive excel sheet that can organize each of our 26 community partners' contributions into a distinct list of referral pathway resources made available to the patients upon discharge from the Mental Health, Addictions, Substance Use Health Urgent Care Center.


Potential for Impact: This project is essential in its contribution to how the clinic will operate through the collaboration of 26 community partners and through its ability to identify what types of resources can be utilized to provide the right care at the right time for whole-person care.
